1. Online Appointment
After completing the online appointment form, you will receive an email to confirm your appointment. Click CONFIRM to receive another email with the application form and instructions. Print the form.
A confirmed appointment is strictly required; no walk-ins will be processed.
Appointments are free. Report any person or group charging for slots.
Ensure your email address is correct to receive the confirmation and instructions.

Requests are subject to approval and availability. Appointment rescheduling is free.

Accomplished application form (sent via email)
Current Philippine passport – original and 1 photocopy of the data page
Valid Singapore pass (IC, EP, S Pass, Work Permit, etc.) – original and 1 photocopy
Short-term visit pass holders: photocopy of STAMP on current passport
Note: Additional requirements apply for minors, dual citizens, and applicants requesting changes in passport entries.
