- All clients for PHL citizenship renunciation are required to make a prior appointment through the online booking system of the Embassy.
- Personal appearance of the applicant is required during submission and collection of documents.

Accomplished application form
Original and one (1) copy of ICA’s letter requiring renunciation of Philippine citizenship or indicating that the application is already at its final stage
Two (2) recent, identical passport-sized photos (4.5 cm x 3.5 cm)
Current Philippine passport and one (1) photocopy of data and signature pages
Embassy processor will provide two (2) Oath of Renunciation forms during application
Processing Fee of S$75.00, to be paid in Cash or PayNow
If the passport is lost, applicant must submit an Affidavit of Loss (notarization fee: S$37.50) and present the original copy of PSA Birth Certificate and submit one photocopy of it.
If never issued a passport since birth, applicant must execute an affidavit stating the same and present the original copy of PSA Birth Certificate and submit one photocopy of it.
