DOCUMENTARY PROOF OF FILIATION
What are the documents needed to prove filiation?
- For foreign spouse of Filipino national:
- Marriage certificate*. Original Marriage Certificate (if married in the Philippines) or Report of Marriage (if married abroad) from the Philippine Statistics Authority (PSA), or copy of Report of Marriage (ROM), if married abroad within the past one (1) year and the PSA copy is not yet available; and
- Passport of spouse. Copy of valid passport data page of the Filipino spouse
* Certificates on same-sex marriages and common-law spouses are not acceptable at this time as these are inconsistent with Philippine regulations
- Foreign minor children, and foreign children with special needs regardless of age, of Filipino nationals:
- Birth certificate. Original PSA-issed Birth Certificate (if born in the Philippines) or Report of Birth (if born abroad), or Copy of Report of Birth (ROB), if born abroad within the past one (1) year and PSA copy is not yet available
- Medical certificate. Authenticated medical certificate and other authenticated documents to prove special needs
- Passport of parents. Copies if data pages of valid passports of the Filipino parents
- Foreign parents of minor Filipino children and of Filipino children with special needs regardless of age
- Birth certificate WITH the foreign national indicated as parent. Original PSA-issed Birth Certificate (if born in the Philippines) or Report of Birth (if born abroad), or Copy of Report of Birth (ROB), if born abroad within the past one (1) year and PSA copy is not yet available;
- Medical certificate. Authenticated medical certificate and other authenticated documents to prove special needs of the Filipino child; and
- Passport of child/ren. Copies if data pages of valid passport of the Filipino child with special needs
The Embassy reserves the right to deny requests due to incomplete requirements.
