- This visa is issued to the entire crew of a registered vessel or aircraft entering the Philippines. Crew members not listed on the Crew List Visa or without a valid individual Philippine visa will be denied entry.
To apply, email your request with the completed application form and required documents to crewlist.sgpe@gmail.com. Use the vessel name, estimated arrival date in the Philippines, and name of the authorized representative as the email subject.

Supplemental Form [download form here] accomplished by the Vessel Master/Captain [downloadable from the Embassy website]
Scanned FA Form No. 2 with photo – one per foreign crew member [download form here]
Three printed copies of the Crew List Visa application form
Digital copy of the accomplished Crew List Visa application [must also be emailed]
Scanned passport data page of each crew member [passport must be valid for at least 6 months from estimated departure from the Philippines]
Signed letter from the vessel owner naming the Vessel Master/Captain and authorized ship agent, requesting issuance of a Crew List Visa; include supporting documents for multiple port calls [if applicable]
Copy of the Vessel Master/Captain’s passport [must be valid for at least 6 months from departure from the Philippines; print on A4 paper]
Copy of the Vessel Master/Captain’s Seaman’s Record Book, showing the latest log entry [print on A4 paper]
Copy of the vessel register showing vessel name, IMO number, and specifications
Visa fee payment [cash only; no checks/cards/NETS]:
Up to 40 crew members: SGD 187.50
41–100 crew members: SGD 262.50
101–200 crew members: SGD 337.50
Over 200 crew members: SGD 412.50
Instructions
The Crew List Visa application form (MS-EXCEL format) is downloadable from the Embassy website. It contains two tabs:

Complete list of crew members and passengers
Instructions for Completing the Form:
Download the official form from the Embassy website.
The Local Agent may fill out the form.
Enter all data directly into the digital Excel file [no handwritten entries].
Use the Vessel Master/Captain’s passport details, even if they hold a valid Philippine visa or are Filipino.
Print three (3) copies on A4-size paper.
The Vessel Master/Captain must review and sign Section F [Declaration] on all printed copies.
Email the actual Excel file [not PDF] to crewlist.sgpe@gmail.com with the subject: “CREW VISA APPLICATION.”
The Local Agent must:
Bring the three printed copies to the Philippine Embassy in Singapore.
Sign Section G [Attestation of Agent] in front of a consular officer.
Pay the required visa fee in cash.
The Local Agent receives the approved Crew List Visa and provides it to the Vessel Master/Captain for presentation to Philippine immigration upon arrival.
