REPORT OF DEATH OF A FILIPINO NATIONAL

The death of a Filipino in Singapore should be reported to the Philippine Embassy by the next-of-kin of the deceased or any person who has knowledge of the facts of death within thirty (30) days from the date of its occurrence. 

REPORT OF FETAL DEATH

The Philippine Statistics Authority defines Fetal Death as the death prior to the complete expulsion of a product of conception, irrespective of the duration of pregnancy, The death is indicated by the fact that after such separation, the fetus does not breathe nor show any other evidence of life, such as beating of the heart, pulsation of the umbilical cord or definite movement of voluntary muscles.

The following are the documents required for the issuance of the ROD, CMC and Certificate of No Objection.

 

I. Documentary Requirements for the issuance of Report of Death or Report of Fetal Death

  1. Certificate of Registration of Death (1 original + four (4) photocopies)
  2. Duly accomplished Report of Death form (1 original + four (4) photocopies)
  3. Passport of the deceased (1 original + four (4) photocopies)
  4. Passport of the deceased’s next-of-kin or informant (four (4) photocopies)

  1. Certificate of Registration of Death, or any document issued by the hospital stating the particulars of the child as well as the circumstances of his/her death (1 original + four (4) photocopies)
  2. Duly accomplished Report of Fetal Death form (1 original + four (4) photocopies)
  3. Passport of the parents (four (4) photocopies)
  4. Marriage Certificate of parents, if married (four (4) photocopies)

Late Registration of Death or Fetal Death – In the event that the death of the Filipino national was not reported to the Embassy within thirty (30) days from the time of death, an Affidavit of Delayed Registration executed by the deceased’s next-of-kin and duly notarized by the Embassy will be required in addition to the documents listed above, whichever is applicable.


* Please note that additional documents may be required upon assessment, depending on the personal circumstances of the deceased and/or cause of death.

II. Shipment of Humans/Cremated Remains to the Philippines

If the human remains or cremated remains of the deceased will be shipped to the Philippines, the Philippine Embassy shall issue a Consular Mortuary Certificate (CMC) and Certificate of No Objection (CNO) for said purpose. For such purpose, submit the following documents:

  1. Certificate of Registration of Death (original)
  2. Duly accomplished Report of Death form (original)
  3. Permit to Export Coffin (1 original + four (4) photocopies)
  4. Embalming Certificate (1 original + four (4) photocopies)
  5. Sealing of Casket Certificate (1 original + four (4) photocopies)
  6. Passport of the deceased (1 original + four (4) photocopies)
  7. Passport or ID of the informant (1 original + four (4) photocopies)
  8. Medical Certificate from hospital or Health Sciences Authority indicating that the deceased was not infected with COVID-19 or did not die of a COVID-19-related disease (1 original + four (4) photocopies)
  9. Confirmed flight itinerary for the shipment of human remains/cremated remains of the deceased to the Philippines
  10. Full name, complete address, and contact details of the designated consignee in the Philippines

    3. Application for Cremation (1 original + four (4) photocopies)
    4. Certificate of Collection of Cremated Remains (1 original + four (4) photocopies)
    5. Passport of the deceased (1 original + four (4) photocopies)
    6. Passport or ID of the informant (1 original + four (4) photocopies)
    7. Confirmed flight itinerary for the shipment of human remains/cremated remains of the deceased to the Philippines
  • Full name, complete address, and contact details of the designated consignee in the Philippines

III. Procedure and Other Instructions

  1. Submit scanned copies of the COMPLETE SET OF DOCUMENTARY REQUIREMENTS via email to atn@philembassysg.com for pre-evaluation.
  2. Once all the documents are in found to be order, an appointment will be provided via email or SMS to personally submit the COMPLETE SET OF ORIGINAL DOCUMENTS and four (4) sets of PHOTOCOPIES of each of the original documents
  3. Pay the corresponding consular fees at the cashier section.
  4. Release-claim documents – The date and time of the release of the ROD, CMC and CNO will be indicated on the official receipt to be issued after payment of consular fees. No processing and releasing of documents during weekends and public holidays.   

IV. Turnaround Time for the Issuance of ROD, CMC and Certificate of No Objection

  • Additional fees may also be collected for amendment or correction of ROD and CMC, such as changes in the flight details, consignee details, etc.
