Report of Death

The death of a Filipino in Singapore should be reported to the Philippine Embassy by the next-of-kin of the deceased or any person who has knowledge of the facts of death within thirty (30) days from the date of its occurrence. 

Upon being notified of the death, the Philippine Embassy shall issue the Report of Death (ROD). If the human remains or cremated remains of the deceased will be shipped to the Philippines, the Philippine Embassy shall also issue the Consular Mortuary Certificate (CMC) and Certificate of No Objection.

The following are the documents required for the issuance of the ROD, CMC and Certificate of No Objection.

A. Report of Death

  1. Certificate of Registration of Death (original)
  2. Duly accomplished Report of Death form
  3. Passport of the deceased (original)
  4. Passport of the deceased’s next-of-kin or informant 

B. Shipment of Human Remains

    1. Certificate of Registration of Death (original)
    2. Duly accomplished Report of Death form
    3. Permit to Export Coffin (original)
    4. Embalming Certificate (original)
    5. Sealing of Casket Certificate (original)
    6. Passport of the deceased (original)
    7. Passport of the informant, if available (original)
    8. Confirmed flight itinerary for the shipment of human remains/cremated remains of the deceased to the Philippines
    9. Full name, address, and contact details of the designated consignee in the Philippines
    10. Medical Certificate from hospital/Health Sciences Authority indicating that the deceased was not infected with COVID-19 or did not die of a COVID-19-related disease

C. Shipment of Cremated Remains

    1. Certificate of Registration of Death (original)
    2. Duly accomplished Report of Death form
    3. Application for Cremation (original)
    4. Certificate of Collection of Cremated Remains (original)
    5. Passport of the deceased (original)
    6. Passport of the informant, if available (original)
    7. Confirmed flight itinerary for the shipment of human remains/cremated remains of the deceased to the Philippines
    8. Full name, address, and contact details of the designated consignee in the Philippines

D. Late Registration of Death

In case the death of the Filipino national was not reported within thirty (30) days from the time of death, an Affidavit of Delayed Registration (click here to download the form) executed by the deceased’s next-of-kin and duly notarized by the Embassy will be required in addition to the documents listed in A, B or C above, whichever is applicable.

Please submit scanned copies of the COMPLETE SET OF DOCUMENTARY REQUIREMENTS to atn@philembassysg.com for pre-evaluation.

You will be provided with an appointment to submit the ORIGINAL DOCUMENTS once all the scanned documents are deemed COMPLETE and IN ORDER.  

Applicable Consular Fee(s)

  • Notarial fee of S$42.50 for the Report of Death
  • Certification fee of S$42.50 for the Consular Mortuary Certificate
  • Notarial fee of S$42.50 for the Affidavit of Delayed Registration
  • Expedite fee of S$17.00

Payment(s) can be made through CASH or PAYNOW

Turnaround Time for the Issuance of ROD, CMC and Certificate of No Objection

  • Regular processing

Two (2) working days upon receipt of complete and correct documents

  • Expedite processing

Same day release if the complete set of documents is submitted before 11:00 AM

Next working day release (10:30 AM onwards) if the complete set of documents is submitted after 11:00 AM
