(Applicable to infants and children born in Singapore to Filipino parent/s)
Make an appointment through the Embassy’s Appointment Portal.
Documentary Requirements
1. Birth Certificate
2. Report of Birth Form
3. PSA-issued Marriage Certificate or PSA-issued Report of Marriage
4. Father's Passport
5. Mother's Passport
6. Affidavit of Late Registration of Birth
1. Birth Certificate
Issued from Singapore Registry of Births – original and five (5) copies.
2. Report of Birth Form
Download form here – five (5) copies, filled out, typewritten and signed by parent(s) with handwritten or “wet” signature.
3. PSA-issued Marriage Certificate or PSA-issued Report of Marriage
Original and five (5) copies; original must be presented for sighting.
If applying for a Report of Marriage at the same time, please refer to this link for the requirements.
Note:
If the registration of birth and marriage will be done at the same time, kindly set an appointment in the afternoon.
4. Father's Passport
Five (5) copies of data page and/with signature page; original must be presented for sighting.
5. Mother's Passport
Five (5) copies of data page and/with signature page; original must be presented for sighting.
6. Affidavit of Late Registration of Birth
Download form here – required if registration is made one year after child’s birth.

Affidavit of Admission of Paternity (AAP) [download form here] – Accomplished by the father and signed before a consular officer. If abroad, it must be authenticated by the Philippine Embassy/Consulate or DFA (if in the Philippines)
Affidavit to Use the Surname of the Father (AUSF) [download form here] – Accomplished by the mother and signed before a consular officer.

If the registration of birth and filing of AAP and/or AUSF will be done at the same time, kindly set an appointment in the afternoon.
If the child’s surname will be under the mother’s surname, kindly present the mother’s PSA/NSO Birth Certificate.

Newborn’s personal appearance is required for photo/biometrics capture.
FEES (payment may be in CASH or PayNow)
- Report of Birth fee: SGD37.50
- Certificate of Registration fee: SGD37.50
- New passport application fee: SGD90.00
- Legalization fee for Affidavits (AUSF, AAP, late registration): SGD37.50 per affidavit
