Amendment of passport details at the Philippine Embassy  shall be strictly by appointment only.  Please book an appointment through this linkhttps://philippineembassysgpassport.timetap.com/

The change of name (due to adoption, marriage, dissolution of marriage, widowhood) on the passport requires renewal of the passport. The change of name cannot anymore be manually written on the passport, as immigration authorities of many countries do not anymore recognize manually-entered amendments on the passport.

Any change on the personal details on your passport will require the provision of a proper personal identity or civil status documents as evidence and basis for the change.

The Embassy will accept only Philippine identity or civil status documents that are PSA or NSO-issued (printed on security paper) and authenticated (or Apostillized) by the Department of Foreign Affairs. The information on the DFA authentication procedure is published on the DFA website at this link. https://consular.dfa.gov.ph/authentication. Singapore identity papers must be issued by either the Immigration and Checkpoints Authority or the Registry of Marriages.

The Embassy will not amend any Philippine passport issued prior to 01 January 2015 (including the non-machine readable brown or green passports).

Documentary requirements to change surnames

For Women Who Got Married in Singapore

  • Personal appearance of the applicant
  • Accomplished application form for passport renewal
  • Original PSA-issued certificate of Report of Marriage filed at the Philippine Embassy in Singapore

For Women Who Got Married Abroad (In a country other than Singapore)

  • Personal appearance of the applicant
  • Accomplished application form for passport renewal
  • Original PSA-issued certificate of Report of Marriage issued by the Philippine Embassy or Consulate where the marriage was solemnized; this PSA-issued certificate of Report of Marriage must be authenticated by the DFA in the Philippines.
  • Photocopy of the data page of the passport of husband. Bring the original for sighting.
  • Photocopy of the data page of the passport of wife. Bring the original for sighting.
  • Cash or PayNow payment of the passport renewal fee (SGD 102.00)

For Women Who Got Married in the Philippines

  • Personal appearance of the applicant
  • Accomplished application form for passport renewal
  • Original Philippine Statistics Authority (PSA) issued marriage certificate, authenticated (or Apostillized) by the DFA
  • Photocopy of the data page of the passport of husband and wife. Bring the original for sighting.
  • Cash or PayNow payment of the passport renewal fee (SGD 102.00)

For Women Who Were Divorced from Their Foreign Husbands

A foreign divorce must be first recognized by a Philippine Court, under Article 26 of the Family Code of the Philippines. This is obtained only by hiring a lawyer in the Philippines to lodge a petition for judicial acknowledgment of foreign divorce with a Philippine Court. If the petition is successful, a Court Order will be issued for the purpose of amending the applicant’s PSA marriage certificate.

  • Original and photocopy of Amended PSA marriage certificate (with notation of dissolution of marriage), authenticated (or Apostillized) by the Department of Foreign Affairs (DFA).   • Original and photocopy of the applicant’s PSA birth certificate, authenticated (or Apostillized) by the Department of Foreign Affairs (DFA). The birth certificate is needed as the name on the passport will be reverted to the applicant’s maiden name.
  • Photocopy of the data page of the applicant’s current passport. Bring the original for sighting.
  • Cash or PayNow payment of the passport renewal fee (SGD 102.00)

For Women Whose Marriage Was Annulled by a Philippine Court

  • Personal appearance of the applicant
  • Accomplished application form for passport renewal
  • Original and photocopy of Amended PSA marriage certificate (with notation of nullity of marriage), authenticated (or Apostillized) by the Department of Foreign Affairs (DFA).   • Original and photocopy of the applicant’s PSA birth certificate, authenticated by the Department of Foreign Affairs (DFA). The birth certificate is needed as the name on the passport will be reverted to the applicant’s maiden name.

  • Cash or PayNow payment of the passport renewal fee (SGD 102.00)

For Widows/Widowers

  • Personal appearance of the applicant
  • Accomplished application form for passport renewal
  • Original and photocopy of PSA death certificate of deceased spouse, authenticated (or Apostillized) by the Department of Foreign Affairs (DFA). Information on the authentication procedures at the DFA may be found in this link: https://consular.dfa.gov.ph/authentication) If the spouse passed away in Singapore, the death certificate or death extract issued by ICA, and legalized by the Singapore Academy of Law.
  • Original and photocopy of the applicant’s PSA birth certificate, authenticated (or Apostillized) by the Department of Foreign Affairs (DFA). The birth certificate is needed as the name on the passport will be reverted to the applicant’s maiden name.
  • Photocopy of the data page of the applicant’s current passport. Bring the original for sighting.

Amendment of Name for Adopted Minors

  • Personal appearance of the applicant
  • Accomplished application form for passport renewal
