Requirements for Submission of Employment Documents for Verification / Authentication (Household Service Workers)


A.  Agency Process / Requirement

New Deployment / Passport Renewal / Regularization / Transfer / Homeleave
(FDW who has served less than 2 years with an employer, the processing will be through an Embassy-accredited Singapore employment agency)

  • Original copy of S$2,000.00 performance bond (Employer’s)
  • Origianl copy of S$5,000.00 performance bond (Agency)
  • 2 original duly signed Standard Employment Contract
  • 2 copies In-Principal Approval from the MOM
  • 2 copies of Agency Undertaking
  • 2 copies of Employer Undertaking
  • 2 copies of Prepaid Ticket Advice (PTA)
  • 2 copies of worker’s passport
  • 1 copy of medical insurance
  • 1 copy of employer’s IC
  • 1 copy of Philippine agent’s POEA license

B.  Name Hire

Balik Manggagawa (Passport Renewal / Regularization / Transfer / Homeleave
(FDW who has served continuously for at least 2 years with the same employer)

  • Original copy of S$7,000.00 performance bond
  • OWWA
  • 2 original copies duly signed Standard Employment Contract
  • 2 copies of Undertaking of an Employer
  • 2 copies of worker’s work permit (3 copies if passport)
  • 1 copy of medical insurance
  • 1 copy of employer’s IC

Fees

  • Authentication of Contract – $ 42.50
  • Verification of Contract – S$17.00
  • OWWA Membership – S$35.00
  • Overseas Employment Certificate – S$3.00
  • Pag-IBIG Monthly Contribution – S$5.50
  • Total – $110.00
