Requirements for Seaman / Seafarer Visa

Personal appearance of the applicant
Duly accomplished visa application form
One (1) recent photograph, with white or light-colored background
Applicant’s national passport valid for at least six (6) months or more from date of departure from the Philippines or completion of trip
If the applicant is a seaman joining ship, the company concerned should provide a letter indicating the name of the vessel he will be joining in the Philippines
Cash payment for application processing (refer to consular fees)

Additional Requirements for Crew List Visa for Vessels
